Shipping Samples to the FGSR

Please follow these instructions to ensure that your samples reach the FGSR safely

  1. Follow the tube labeling and order submission instructions on the Submission Guidelines page
  2. We recommend that external investigators submit about 20% more sample material than is required. This can save multiple shipments back and forth if our concentration reading is lower than yours
  3. Fill out and sign the Hazardous Materials Declaration Form. The form can be completed and printed in Adobe Acrobat Reader.
  4. Pack your samples in dry ice, following the diagram below.
  5. Enclose the completed and signed Hazardous Materials Form. If this form is not included with the samples, they will be discarded without further notice.
  6. Print and enclose the Sample Submission PDF that was emailed to you when you placed your order online. This will serve as a packing list - make sure that the number of samples enclosed matches the number on the form.
  7. Clearly print your project number on the outside of the box
  8. Send the package via a commercial overnight delivery service to:
